To become a Max's Mates, junior supporters club member, all you need to do is ask a parent/guardian (politely!) to complete this APPLICATION FORM and send it to us, together with the £15 membership fee.

Max’s Mates Terms and Conditions
• Max’s Mates is open to those aged Under 16 on September 1, 2006.
• Membership runs annually from September 1.
• Membership of Max’s Mates is non-transferable and cards are personal to each individual member.
• In the event that a membership card is lost or stolen, a duplicate card is available at a cost of £2 plus 50p postage and packaging per card.
• Contents of the membership package are subject to availability; Worcester Warriors reserve the right to alter contents of the package at their discretion.
• Membership packs will need to be collected from the community department during office hours. If you cannot collect your gift, it can be posted to you at a cost of £3 per member.
• WRFC Ltd reserve the right to vary or suspend these terms where circumstances dictate such action is necessary.
